Healthcare Home

Helping adults and children lead healthier lives can greatly reduce the negative impacts of living with long-term mental illness, substance abuse and medical conditions. With Healthcare Home (HCH) we provide coordination of comprehensive primary physical care to Medicaid patients with behavioral health and/or chronic physical health conditions in a way that recognizes all of their needs as persons, not just clients. This is done using a partnership or team approach between the Healthcare Home staff, clients, families and Primary Care Providers.

Service Details

For those who may benefit from the services of our Healthcare Home program, a Nurse Care Manager is assigned to your case based upon your medical needs. The Nurse Care Manager will have contact with you as much as necessary to help you maintain your physical health and wellness.

Your Nurse Care Manager is available to:

  • Help you identify and achieve your healthcare and wellness goals
  • Assure you are receiving the preventive and primary care you need to help manage your chronic physical health conditions
  • Provide or arrange for appropriate health and wellness education related to your general medical and chronic physical health conditions
  • Assist in helping you access a family doctor or other medical providers and coordinate care
  • Assist with your transition home after a hospitalization by reviewing your medications with you and helping you understand your discharge instructions

Nurse Care Managers who focus on the medical needs of behavioral health clients have proven to be very successful in addressing general health issues, as well as in improving outcomes and quality of care.

HCH works with youth and adults.

HCH strives to provide services as conveniently as possible for the client. This means services take place in medical clinics, residential treatment centers, and various other locations where clients receive other forms of care. Many times, appointments are coordinated with caseworkers to ensure transportation and access.

  • Clients must have active Medicaid to qualify for Healthcare Home.
  • Burrell helps clients check for eligibility for Medicaid and assists in enrollment when needed.

As part of our approach to providing the best care for our patients and more importantly our community, we are providing a series of open, free-to-the-public, town hall-style talks with our very own Doc, Dr. Karl Shanstrom.

Once a month on the fourth Friday of every month from 9:30 a.m. to 10:30 a.m. and 12:00 p.m. to 1:00 p.m., join us via Zoom where we will discuss various health conditions such as high blood pressure, diabetes, osteoarthritis, and COPD.

Each of these talks will go over the health issue basics as well as common treatment options, medication education, and ideas for discussions to have with your family doctor. Each talk will be followed by a question and answer session where you can ask the doc and have that talk you always wanted but didn’t have time for.
