Homeless Services

Burrell has significant resources available for homeless clients. In addition to general case management, we also offer a PATH program that provides assistance in transition from homelessness.

Path Program Overview

This is a grant funded team that delivers outreach and services to individuals with serious mental illnesses and those with co-occurring substance use disorders who are experiencing homelessness or at imminent risk of homelessness. Our approach is not moralistic; it is client centered and is adapted to meet the needs of each person. We provide outreach through various community partners as well as on the street. The PATH team provides guidance, support, and access to resources to assist individuals in overcoming the barriers that prevent them from achieving mental health and housing stability.

Cost & Eligibility Details:

PATH case management does not require insurance. The process begins with a screening for determination of eligibility. If the individual is in need of support to obtain medications, actively enrolled PATH clients will be evaluated for temporary medication payment assistance.

Homeless Services

Burrell provides mental health rehabilitative and holistic treatment case management through a supportive team approach which focuses on those that are currently experiencing homelessness or are at a higher risk of experiencing homelessness. The homeless service team works as a collective unit to help individuals attempt to stabilize in mental health and guide treatment through client driven goal sets to reduce barriers and increase functionality in daily living.

Cost & Eligibility Details:

An individual must have Medicaid for CPRC case management services. If the individual is in need of support with temporary assistance with medications and services they may be evalu are evaluated on a case by case basis through POS funding for CPRC-enrolled clients.

  • Engage in case management services
  • Obtain support in securing health insurance
  • Get assistance in acquisition of primary healthcare and mental health providers
  • Participate in needs assessment and referral to appropriate providers and resources
  • Obtain assistance in acquisition of basic living necessities (i.e. food, clothing, housing)
  • Get education and support for basic living skills
  • Participate in process for income-based supports such as connecting with advocates for vocational rehabilitation, resume preparation, disability application, etc.
  • Receive help accessing important documents and elements for financial assistance, employment and housing such as securing a personal identification, birth certificate, deposit for housing, etc.
  • Screening and assessment
  • Identify mental health symptoms and triggers
    • Support in mental health appointments and communicating need
    • Support with medication management and compliance
  • Identify barriers in daily living and functioning
    • Support with basic necessities and referral to community resources
    • Support with developing coping skills and implementation of coping skills
    • Set goals and modify goals to meet client need
  • Advocate and support with navigating local resources and applying for Medicaid, Disability, Employment Services, Housing supports, probation and parole appointments
  • Set up holistic care through Primary Care resources, engaging client in Healthcare Home services, and other specialty services that impact physical health needs.
  • Referral to addiction recovery treatment

Homeless services does not have emergency housing or long term housing, but will support clientele with referrals to housing resources.

The fastest route to services is through our Walk-in Clinic.

Individuals who are currently experiencing homelessness and have severe and persistent mental health needs are encouraged to go to the Burrell Behavioral Health Walk-in Clinic at the Main Center Campus to be screened for services.
